Parent/Guardian Consent and Authorization Agreement
Liability Release and Emergency Medical Consent
I give permission for my child (previously listed) to attend all RAISE28 programs and events. I understand that although RAISE28 staff will supervise my child, I do assume the risk in my child’s participation in the event(s) on or off campus. I acknowledge that I will not seek to have RAISE28 held liable in the event that any accident, injury, illness, loss of property, or any other circumstance or incident occurs during or as a result of my son’s/daughter’s participation in the program. This release of liability includes accident, injury, illness, loss of property, or damages to the student, as well as, to other individuals or property, which may result from the child’s participation in the event. I hereby release and agree to hold harmless RAISE28, employees, and volunteers from any claims arising out of my son’s/daughter’s participation in the event(s). I have read and understand and accept all of the statements recited above and accept full responsibility as described. In case of an emergency, I give permission for my child to receive first aid/any medical treatment needed.


Media Release Authorization
RAISE28 may choose to take pictures or record participants in the programs and activities. These images may be used for RAISE28 displays, brochures, newsletters, archives, news releases, social media, and our website. I hereby grant permission to RAISE28 to take and reproduce photographs and recordings for publication, including publication by news sources and other sources for all educational, trade, advertising and other purposes as determined by RAISE28. You may choose to opt-out of this Media Release Authorization by indicating below ("Additional Information").



Transportation Permission
I grant permission for RAISE28 staff or approved volunteers to transport my child to and from any RAISE28-sponsored events, whether held at the center or off-site. This includes transportation to and from home, school, or other approved locations associated with RAISE28 programming. You may choose to opt-out of this Transportation Permission by indicating below ("Additional Information").
